As a convenience of our customers, SureTest Laboratory gives the option of using a credit/debit cards for payment of services performed. Our policy requires that our private pay or co-pay customer's pay for lab tests and services in full before testing begins. Contact our friendly staff today and they will take care of all your billing information. We understand the in-convenience of not paying on-line but we would like to give you ease of mind that your information is secure and fully protected.
To pay with a credit card: Download and fill out your requisition then scan and email form to Deana, SureTest Laboratory's representative. Her email address is deana@suretestlaboratory.com. Or fax form to 1-800-891-5451 attention Deana. She will contact you for payment information. When the transaction is complete an email receipt will be sent to you for your records.
