Medicare/Medicaid:

SureTest Laboratory, LLC services a large number of Traditional Medicare/Medicaid patients. You can use our Laboratory and Phlebotomy services to collect and test following your physician's requests. All we need is a physician's name and blood request order to obtain the specimens. SureTest Laboratory is a “Fee for Service” Provider with both the Medicare/Medicaid Agencies. We comply with the rules and regulations they demand. This ensures that you that by choosing SureTest Laboratory, you are getting quality Laboratory and Phlebotomy Services. SureTest Laboratory, LLC is contracted with many medical insurance companies. Services for insurance companies vary due to the type of insurance you have. The following is a list of companies we honor.

Medicare, Medicaid, Railroad Medicare, Humana, Amerigroup, Superior Health, Molina Health, Tricare, United HealthCare and all insurance that subside with them.

Note: All insurance, other than traditional Medicare, carries an additional charge due to mobile services. Please contact us with your insurance information for verification from your carrier.

Please advise what tests and services your insurance company doesn't cover. You will be responsible for any fees not paid by your carrier.
